Form: Submit Emergency Contacts

It is very important for all homeowners to submit emergency contacts so that we have this on file and readily accessible in case:

- your home has damage, has been broken into, is on fire or has a gas leak, pipes have broken, alarm is going off or otherwise.

- your vehicle has been broken into or is damaged by an accident, is in danger of towing if not immediately moved.

There could be other issues you may need to be contacted about, or if in the event of your suspected death or other emergency, we have someone to contact.

IF THERE IS SOMETHING GOING ON WITH YOUR HOME THAT REQUIRES INTERVENTION AND WE CANNOT REACH YOU, AS IN BROKEN PIPES, ALARM GOING OFF, OR OTHERWISE, THE FIRE DEPARTMENT AND POLICE WILL BE NOTIFIED.
RESPONSIBILITY FOR DAMAGE TO YOUR HOME BECAUSE OF INTERVENTION IS YOUR RESPONSIBILITY AND NOT THE ASSOCIATION'S.

# of adults, children, pets are asked by fire department when they arrive for an emergency, and in the event of fire it can help them to know if possible lives could still be there.

NOTE,

Adult children can be added for notices to E-Blasts and other news if the owner of the home submits a letter by mail to ACS West directly. This may be needed if the homeowner's adult child(ren) maintain the home or make assessment payments. If it is a matter of an adult child having a power-of-attorney, they need to contact ACS West by mail about this.
